The team will use a survey to collect data on the number of falls after the implementation. The implementation team will use surveys and questionnaires to collect data. Besides, the pre-and-post implementation plan will entail having data before and after the proposed project’s execution to determine its effectiveness (Roberts et al., 2020). The nurse manager will collect data in collaboration with each shift nurse. This implies that on every shift, the nurse in charge should collect data by decoding it once they have done the hourly rounding on each patient (Ryan et al., 2022). The nurse fills the patient chart with data based on what they observed and also self-reports from patients.
2. To enhance patient care experience and interactions with providers Improved quality care to get better reimbursement from CMS. The team will also incorporate patient satisfaction questionnaires to determine the level of satisfaction by patients and their caregivers about the interventions.
