The clinical site for my practicum is Professional Home Health, a hospice that provides home care services in Dallas-Fort Worth. The organization boasts of its established history in providing exceptional care to patients with a wide range of healthcare needs. Among the patient care services offered by Professional Home Health include: Ongoing or intermittent nursing care; Medication management and monitoring; I.V. Infusion Therapy; Cardiovascular Rehabilitation; Wound Care; Oncology Care; Gastrostomy, Colostomy, and Nasogastric Care; Diabetic Care / Insulin Pumps; Care for Urinary Catheter; Neurological Rehabilitation; Orthopedic Rehabilitation (Professional Home Health Care, n.d.). The clinical site is thus ideal for DNP students since they can utilize evidence-based practices in clinical practice to improve the quality of care and health outcomes of patients. Besides, the DNP student can learn about interprofessional collaboration in clinical care since the organization comprises health providers from different professionals, including nurses, social workers, physical, occupational, and speech therapists.

The preceptor for the clinical practicum is Bola Adeola, a Psychiatric and Mental Health Nurse Practitioner (PMHNP) with a Master of Science in Nursing in Psychiatric and Mental Health at Walden University. Bola currently works as Board Certified PMHNP at Telecare Corporation in San Bernardino, California (Walden University, n.d.). She has vast clinical experience working in various patient care settings, including Skilled Nursing Facilities, Home Health, Hospitals, Case Manager in Private Health Establishments, Insurance, Counties, and States.

My specialty is Psychiatric and Mental Health Nurse, which seeks to equip students with advanced education and skills in providing mental health and psychiatric care to individuals in the community. The problem that I have identified and will pursue throughout the course is Diabetic Ulcers. I seek to implement a staff education program to improve nurses’ knowledge and skills in managing patients with diabetic ulcers. The clinical problem and proposed intervention were informed by the rising cases of diabetic patients developing diabetes ulcers and the morbidity associated with the complication (Innes-Walker et al., 2019). It is thus essential to improve nurses’ knowledge and skills in managing the complication.
